CERTIFICATE OF CATHOLIC PRACTICE
INFANT ADMISSIONS 2023
If you require a certificate of Catholic Practice please email your child’s name, date of birth and full address to shirleybailey@rcdow.org.uk stating which church you attend.
Fr Kim will sign the forms and Shirley will email you when they are ready to be collected. Requests for CCP opened on Tuesday 1 November and will close on Monday 21 November.
The closing date to collect certificates will be Thursday 1 December 2022.
